Contacting publishers in search of missing cards

I've received two decks recently that have had duplicates of one card, but are missing another card. I want to write to the publishers, but I'm not sure quite how to do that. Should I try to locate an e-mail address, or would plain paper mail be better? Should I write to the publisher or the distributor, when two different companies are given? To whom should I address my letter? I was taught "Dear sir or madam," but every time I use that it sounds stuffy to me, as does "To whom it may concern."
I know a lot of people have written to publishers about their incomplete decks, so hopefully someone can answer my questions.

I contacted Llewellyn and used the address on the inside flap of a book. I hand wrote them saying I would like a new 9 of swords from such and such deck by the author. For Dear Sir or Madam, I used Dear Revered Publisher,.. hehehhe Sucking up yes, but it can't hurt. ;) It took about a week for them to send me my card so I was delighted at their promptness. They also sent me a catalog! hehe Now I get to order all kinds of neat books now.

My experience was with Bear & Co (the Phoenix deck) and I e-mailed them. They responded very quickly with a whole new deck and didn't even ask for the old one back. I was very impressed with their service.

My experience was with the Cosmic Tribe. I had a marred card and e-mailed the publisher. I used "To whom it may concern" and I received a brand new deck/book set. I was very pleasantly surprised! Remember, publishers know we have many choices, they are pretty responsive and willing to provide good customer service. Good luck!

I got a Tarot Nova with three cards missing and I simply looked up the publisher's website and sent them an email about it. At first they didn't reply and it turned out they never received my first email. When I finally did get a reply, after having sent another email, they immediately sent me the missing cards and I got them within a week.

My vision quest tarot had no cards missing but some cards were misprints. The wantde to send me replacements but the deck was in reprint. I almost forgot about them until i received a email that my replacements would be send to me early january!!!!!!!! :)
