How do you interpret "trouble" cards like 3, 5 swords or 5,7 holes in "wants" or "needs" position? I guess nobody wakes up thinking - I wish someone break my heart today!

I think that mostly I see it as a clue as to what I'll be in want of.
Swords - Needing more communication, information or interaction.
Pentacles - Needing something concerning my health or home, maybe I need to take better care of myself or keep an eye out for possible problems? "If it hurts or just feels wrong, check it out or atleast take it easy" - kind of warning.
Cups - Needing appreciation, to have someone show they care, to maybe need to accept the pain of a loss or loneliness.
Wands - Needing to regain some strenth and inspíration.
Shortly, I see it as an indication on what area you should focus on and maybe I would draw a helpcard to clarify if the card did not tell me something direct like "accept the heartache already" or "ok, feel hurt today, take the time, but later, you'll have a chance to get back on your feet".

The question also recalls the paradox inherent to the Hopes and fears position. Success is desirable but also feared. There is security in the repeating patterns that we've developed, since creation leads to the unknown - and the unknown is feared.
There is a major contradiction between the conscious desires of the ego and the 'real' desires that run deeper and stronger in the unconscious mind.
We humans keep digging traps for ourselves to fall into - usually within very similar situations - thats what Freud called "repetition"
The tarot is a perfect tool to detect such patterns of behavior.

Aun makes an excellent point. Also you need to remember that the 3/Swords doesn't always indicate that YOUR heart will be broken. 3/Swords is about piercing words being said, usually words that someone has kept inside for a while.
Thus, you may WANT and NEED for someone to finally speak up. You've been in that position before haven't you, where you keep saying to that friend of yours, "You have to tell him!" (or her). Whoever "him or her" is--another friend, the boyfriend/girlfriend they need break up with so they can be with you, or just the nice person who keeps joining you for lunch when you want to be alone!
"You have to TELL HIM!" That what YOU want and need, for them to tell this person some piercing but necessary words, get it out there, and, well, let out the poison that's building up. It's not a happy situation, but it's necessary.
It's all how you look at the cards. 5/Swords might not be about you losing arguments, but rather just wanting/needing to not have any more losing arguments. And 5/pentacles might be about wanting/needing to not have to worry about debts and poverty anymore.
That help?

That really helped! Though I´m not sure that I won´t need another explanation for such card in this position })
And Thirteen, aren´t you some kind of teacher? You´re so great in explaining!

What a great point of view for this card!
When it comes to Swords in relationships, the first thing that comes to my mind is 'communication' between the parties... of course it could mean other things, but basics are basics....
