This vampire is going through an old ritual which consists of partially getting rid of their new blood and having it replaced with old blood coming from an old vampire. Not only it's a long and painful process, but the young vampire may die, and he has to find as well an old vampire willing to share the powers that come with long age which is not easy. There are two more crosses in the background which means it's a sacred place devoted solely for this. The vampire here has been there for so long roots have grown and attached themselves to him. He seems to be wounded on his chest close to his heart and neck and the blood is pouring out from his mouth. The snake is a symbol of transformation.

I want to point that the snake is the animal representing Grails, the water element, and with neptune representing water it fits well with the card.
