Tarot Spells by Janina Renee does not list an affirmation per card but contains affirmations together with selected Tarot cards for visualising and affirming a specific outcome.

(In my mind, a written spell is an affirmation of a desired outcome.)

Affirmations for the Everyday Goddess by Pamela Wells

Note that this is a majors only Tarot deck. I just ordered it from amazon US marketplace as it doesn't seem to be available anywhere else in the world (with overseas shipping).

Although the title mentions Goddess, the wording of the affirmations seems less Goddess and more God/Divine which one reviewer objected to.

I have James Ricklef's book that is a companion to his deck The Tarot of the Masters, and I find it to be very good, and also easily used with other decks. He has several affirmations for each card.

The Gaian Tarot book also has an affirmation for each card.
